How do I request a copy of my medical records?

Requesting medical records involves three simple steps: 

  1. Print the Authorization for Release of Information (ROI) form. 
  2. Complete and sign the form. 
  3. Submit the form by fax, email, mail, or in person.

Is there a fee to obtain medical records?

No. There is no cost to receive copies of medical records from Capitol Park Mental Health Hospital.

What are the visiting hours at Capitol Park Mental Health Hospital?

Unit 3
Visitation: Monday, Wednesday and Saturday: 11am – 11:45am
No visiting hours Sunday, Tuesday & Friday

Unit 4

Visitation: Tuesday, Thursday and Saturday: 1:30pm – 2:30pm
No visiting hours Sunday, Monday, Wednesday & Friday

Visiting hours may change based on community needs or physician-ordered restrictions. 

What time should I arrive for visitation?

Arrive at least 15 minutes early to complete the check-in process. 

Are children allowed to visit patients?

No. Visitors must be age 18 or older. For safety and to maintain a therapeutic environment, minors are not permitted. Children are encouraged to stay connected via phone. 

How many visitors are allowed at one time?

A maximum of two (2) care partners may visit at a time. Larger groups should stagger visits to avoid crowding.

Can I bring a cellphone or smartwatch into the hospital?

No. Personal electronic devices are not allowed. This includes cellphones and smartwatches, which may compromise patient privacy. Please leave electronics in your car. 

Can I bring outside food or drinks for a patient?

No. Outside food and drinks are prohibited by the health department. The hospital provides balanced meals and snacks for all patients. 

Who is allowed to visit a patient?

Only approved visitors may visit. Visitors must be listed on the patient’s visitor/phone consent list. Patients may refuse visitors at any time by informing staff.
