Tabitha Farr, MSW, LICSW
Director of Quality
Tabitha Farr brings extensive expertise in mental health, long-term care, program development, and healthcare leadership to her role as Director of Quality. She holds a Master of Social Work from the University of St. Thomas and is a Licensed Independent Clinical Social Worker in Minnesota (LICSW) and a Licensed Clinical Social Worker in Wisconsin (LCSW). Her professional background spans direct clinical practice, regulatory compliance, and organizational leadership, all grounded in a strong commitment to advancing high-quality, patient-centered mental health services.
Throughout her career, Tabitha has been driven by a deep passion for healthcare advocacy, particularly for individuals who may not be able to advocate for themselves. She has guided organizations through complex regulatory environments, developed and strengthened policies and procedures, and built operational practices that support long‑term regulatory excellence. She believes that maintaining a constant state of survey readiness is not merely a compliance goal but a meaningful demonstration of an organization’s commitment to exceptional care and improved patient outcomes.
Tabitha is dedicated to fostering a culture of empathetic accountability, continuous improvement, and compassionate care. By integrating her experience in clinical practice, program development, and leadership, she works to advance Capitol Park’s mission of providing safe, effective, and person-centered services.
Outside of work, Tabitha enjoys spending time with her family, traveling and seeking new experiences, cooking and gardening, and exploring new restaurants and cuisines—often attempting to recreate her favorite dishes at home.
